A hospital in East Ayrshire has been ordered to improve the way it handles patient safety incidents in its maternity unit, almost 10 years after it was the centre of an investigation into baby deaths. The report from Healthcare Improvement Scotland (HIS) made 16 requirements for improvement at the unit in Crosshouse University Hospital, near Kilmarnock, including for delays women faced when they contacted the triage unit.

It said some staff were reluctant to report patient safety events, and reviews into incidents took too long to the detriment of families. NHS Ayrshire and Arran said it is committed to ensuring patients receive safe and dignified care at all times. In 2016, a review was ordered into failures of care at the hospital after BBC Scotland News revealed there had been six “unnecessary” baby deaths.

The actual Healthcare Improvement Scotland inspection published today based on a visit in October 2025 resulted in 10 areas of good practice, two recommendations and 16 requirements. It makes no reference to the baby deaths 10 years . A Scottish Government national review followed and there have been no further incidents of that kind.

The inspection report does not indicate any concerns that would require a formal inquiry or police investigation. While the report highlights several areas for improvement, such as delays in maternity triage, gaps in incident reporting, fire safety compliance, and training deficiencies, these issues are categorized as requirements for improvement rather than criminal matters. 

In England four trust maternity units, alone, are subject to police investigations. 1, 2, 3, 4

Additionally, a broader national maternity and neonatal investigation (led by Baroness Amos) is examining 12 other trusts for similar failings (e.g., high perinatal mortality).5

In Scotland, six boards being monitored

The correct term in Scotland is ‘Support and Intervention Framework (escalation levels)‘ and no Scottish board is at level 5 indicating any safety concerns for patients. There are six being monitored at lower levels for, eg, financial problems, but none for patient safety, including waiting times, issues.6

      Here is another example that happened recently.

      January 2025

      Angus Robertson wrote a FORMAL COMPLAINT to the BBC Director General on a lie that was broadcast on BBC Radio 4 broadcast in respect to the Scottish Budget.

      This then prompted the BBC to publicly apologise on air for the (supposed) ‘mistake’ that they made on their radio news programme.

      What was the lie ?

      They had broadcast that “the Scottish budget would mean more than half Scottish workers would pay more tax than if they lived elsewhere in the UK

      That was a lie.

      The actual reality aka truth was that………..

      The tax change announced in Holyrood will actually lead 55% of workers in Scotland paying less tax compared to other parts of the UK.

      That’s some ‘mistake’ to make , indeed they broadcast the opposite of what was actually the truth.

      So I then wonder what exactly is the purpose of this BBC ‘Verify’ Unit that exists ?

      Well the BBC states that “BBC Verify is a specialist team of journalists using open-source intelligence (OSINT), satellite imagery, data analysis and forensic techniques. They fact-check information, verify video, counter disinformation, and analyse data to separate fact from fake, and to bring clarity on complex issues

      So then just what are they verifying in the news that they themselves produce and broadcast in Scotland and elsewhere in the UK.

      As it is , at times, certainly not the factual ‘news relating to where we are’.

      Nor is it news where they have first “fact-checked” the information before broadcasting it in order to try it to counter disinformation being communicated by their news teams.

      Neither have they brought clarity but instead they have generated confusion in communicating a lie and then later having to correct that lie , that they state was merely an error or a mistake.

      (Or rather them being forced to make an on air public correction because of a complaint(s) from an SNP Cabinet Minister- or via clued up members of the public).

      Just think of all of the so called ‘mistakes’ in their previous BBC news broadcasts that were considered to be supposed factual news that may not get noticed by either the public or the SNP.

      So then they , as (fake) news reports, are broadcast to the public and so are then allowed to stand uncorrected as supposed factual news reports, but they were clearly misinformation (lies) when they were broadcast .

      I think it is not the BBC who should have a ‘Verify’ unit to check the facts produced by others as it seems more of a priority that they themselves should begin to start to clean out their own house first.

      Also it is perhaps the SNP communications team who should be verifying the accuracy of the BBC’s news output in Scotland so that they are then always ready , when applicable, to make more FORMAL COMPLAINTS to the BBC Director General and demand a public apology and correction.

      I mean they, the BBC, ducked up with Trump and also Nigel Farage is quick off the mark if the BBC ever misquote him, so then the SNP should also be on the ball as they , the BBC, really are becoming known as those who are more likely to publish and broadcast fake news, especially news concerning the SNP.

      Indeed this very site (and others) often give examples of times when the BBC fall way short in their dodgy news reports upon politics within Scotland and the Scottish government.

      So basically once you are identified and exposed as an unreliable and untrustworthy source of news your influence upon public opinion will start to wane.

      You will then be less able to sway public opinion as you will be considered irrelevant as a news source and so then best avoided.

      Look at the (not widely known) hidden list on the BBC website that is their shameful list of BBC corrections and clarifications , which relates to (fake) news and (mis)information that they , the BBC, have broadcast in their various TV and Radio news programmes , as in statements that should never have been allowed to have been publicly aired as they were not true.

      Ironic then that the BBC seeks to both judge and scrutinise the SNP when surely it is they themselves who need to be judged and scrutinised more upon their appalling record in producing dodgy news reports .
